כיצד מגישים בקשה
ניתן לפנות למרפאת שיניים סגול ולבקש עיון במידע, תיקון מידע שגוי, הגבלת שימוש או מחיקה, בהתאם לדין.

מה לציין בפנייה
- שם מלא;
- מספר הטלפון שממנו נעשה שימוש בבוט;
- סוג הבקשה: עיון, תיקון, הגבלה או מחיקה;
- תיאור קצר של המידע או הפעולה המבוקשת.
אין לשלוח מספר זהות מלא בהודעת WhatsApp ראשונית. המרפאה עשויה ליצור קשר בערוץ מתאים לצורך אימות זהות.
אימות זהות
לצורך הגנה על פרטיות המטופלים, המרפאה רשאית לבקש פרטים נוספים לפני מסירת מידע או שינויו.
מגבלות על מחיקה
מידע הנדרש לשמירה לפי דין, מידע ברשומה רפואית, רישומי תורים, תיעוד הסכמה, חיוב, אבטחה או הגנה משפטית עשוי שלא להימחק מיד.

Include the following
- Full name;
- The telephone number used with the bot;
- The requested action;
- A short description of the information concerned.
Do not send a complete identification number in the first WhatsApp request. The Clinic may use another channel to verify identity.
Deletion limitations
Information required by law or forming part of a medical, appointment, consent, billing, security, or legal record may not be eligible for immediate deletion.
